What a great question! We have never had a guest ask to bring a service animal, but I would imagine that we would be obliged to let it in dorms as well as private rooms. Our unit of sale is the bed rather than the room, so for guests with disabilities there shouldn't be any difference between booking a private room or a dorm in terms of permissions or application of the law.
For guests with allergies or phobias, the only thing to do would probably be to move them to another room. If nothing is available, then perhaps another guest would be willing to trade places. We usually have enough guests who feel pet deprived on their trip that they would welcome the chance to sleep in the room with a service animal.
We do not assign top or bottom bunks. How do you handle guests with disabilities that require a bottom bunk but do not want to book private accommodations?
Again, assume we do not have the open space to accommodate this without personally removing guests from beds they have already claimed.
We have dealt with this, and the easiest solution for us has always been to ask another guest to trade beds. Most people will be happy to move simply because we asked them to, and others will at least be willing to move when they see that the other person isn't able to use a top bunk. Normally we're close enough to at least one person in the room that finding someone to swap beds is easy enough.
